A note for patients seeking mental health medication management

If you do not have an established primary care provider, you will need to be seen as a new primary care patient here at Hope Health and Wellness prior to consideration for a psychiatric evaluation and psychiatric medication management.

If you already have an established primary care provider, please have them submit a referral for psychiatric evaluation and medication management.
